What Is A Kidney Stone?
A kidney stone is a hard deposit formed from minerals, salts and chemicals present in urine. Common stone-forming substances include calcium, oxalate, uric acid, phosphate and cystine.
When urine becomes concentrated because of dehydration, low water intake, dietary factors, metabolic problems or certain medical conditions, these substances can begin forming crystals.
Over time, these crystals can join together and grow into stones. Kidney stones may be extremely small or may grow much larger.
Some stones remain inside the kidney without causing symptoms, while others move into the urinary tract and cause pain, bleeding, urinary blockage or infection.
The National Kidney Foundation explains that kidney stones can form when urine contains high levels of certain stone-forming substances and not enough fluid to keep them diluted.
Why Is Kidney Stone Pain So Severe?
The urinary tract is narrow and sensitive. When a stone moves from the kidney into the ureter, it may partly or completely block the normal flow of urine.
Urine can then build up behind the obstruction and increase pressure inside the kidney. At the same time, the ureter may contract and go into spasm while trying to push the stone toward the bladder.
The combination of obstruction, pressure and ureteric spasm can create severe pain known as renal colic.
Renal colic often comes in waves. The pain may become very intense, ease slightly and then return again as the ureter continues contracting.

Kidney stone pain is not exactly the same for every patient. Its character can change according to the stone's size, location and movement through the urinary tract.
1. Dull Back Pain
Some patients experience mild or dull pain in the back or side. It may feel similar to a muscle pull or sprain.
This can occur when a stone remains inside the kidney without causing major obstruction.
2. Sharp Flank Pain
Flank pain occurs on the side of the body between the lower ribs and the hip.
Kidney stone flank pain can be sudden, sharp and extremely severe.
3. Wave-Like Pain
Pain may become severe for several minutes, reduce and then return again.
This wave-like pattern can happen because the ureter contracts while trying to move the stone forward.
4. Pain Radiating To The Groin
As a ureter stone moves downward, the pain may travel from the back or side toward the lower abdomen, groin, testicle or urinary passage.
5. Burning Urination
When the stone reaches the lower ureter or moves close to the bladder, burning urine, urgency or frequent urination may occur.
6. Cramping Pain
Some patients describe twisting or cramping discomfort inside the abdomen, which may occur because of ureteric spasm.
Where Does Kidney Stone Pain Occur?
The location of kidney stone pain often changes according to where the stone is located inside the urinary tract.
Stone Inside The Kidney
A stone remaining inside the kidney may cause discomfort in the back or flank. The pain may be dull, mild or intermittent. Some kidney stones cause no symptoms while they remain stationary.
Stone Inside The Ureter
Pain can become much more severe when a stone moves into the ureter.
The pain may start in the back or flank and then move toward the lower abdomen or groin. Nausea, vomiting, sweating and restlessness can also occur during a severe attack.
Stone Near The Urinary Bladder
When the stone moves closer to the bladder, patients may experience frequent urination, urinary urgency, burning while passing urine, lower abdominal discomfort or blood in the urine.

Can Small Kidney Stones Also Cause Severe Pain?
Yes. A common misconception is that only large kidney stones cause severe pain.
In reality, a small stone moving through the ureter can produce extreme pain because it irritates the urinary tract and may temporarily block urine flow.
At the same time, a larger stone sitting quietly inside the kidney may sometimes cause very little discomfort.
Is Kidney Stone Pain Similar To Childbirth Pain?
Many patients describe renal colic as one of the worst pains they have experienced. Some compare it with labour pain because both may be severe, wave-like and difficult to tolerate.

When Is Kidney Stone Pain Dangerous?
Kidney stone pain can become more serious when it is associated with urinary infection, severe obstruction or problems with kidney function.

2. Hydration
Adequate fluid intake helps maintain normal urine flow. However, forcing excessive water during severe pain, repeated vomiting or complete urinary blockage is not appropriate.
3. Warm Compress
A warm compress over the painful area may provide temporary comfort for some patients.
4. Medical Evaluation

2. URS / Ureteroscopy
URS is commonly used for selected ureter stones. A scope is passed through the natural urinary passage to reach, break or remove the stone.
3. RIRS
RIRS uses a flexible scope passed through the urinary tract into the kidney, where suitable stones can be treated using laser energy.
4. ESWL
ESWL uses shockwaves from outside the body to break selected kidney stones into smaller fragments.
5. PCNL
PCNL is commonly considered for larger or more complex kidney stones and uses a small tract through the back to access the kidney directly.
How To Prevent Kidney Stone Pain In Future
Prevention depends on the stone type, medical history and reason for stone formation.
